How "best value" is computed.

Each hospital gets two ranks: one by cash-pay price (cheapest = rank 1) and one by CMS overall rating (highest = rank 1). The two are summed; the lowest sum is the best value. Ties broken by price.

Hospitals without a published CMS rating (specialty, pediatric, cancer-only) are excluded from this ranking because the composite would be meaningless. They show up on the procedure overview page instead.
