Out-of-network.

No contract with your insurer.

A provider that does NOT have a contract with your insurance. They can bill you whatever they want, and your insurance typically covers a smaller percentage (or nothing). The No Surprises Act (2022) protects you from out-of-network bills in many emergency and facility-based situations.
